January 1, 2013 - December 31, 2013 Plan Year:

DEADLINE FOR INCURRING CLAIMS:

* Health FSA : March 15, 2014

* Dependent Care FSA : December 31, 2013

DEADLINE FOR SUBMITTING CLAIMS TO ASI FLEX (both Health Care and Dependent Care):

* March 31, 2014 for active employees, or 90 days after termination/retirement for those who have left City employment
